CHES offers two self-directed care programs delivered by registered nurses with DDA-approved training. These programs are Nursing Consultations and Nursing Case Management and Delegation. With the Nursing Consultation program, the RN nurses collaborate with individuals, families, and any relevant staff to establish routines and protocols to monitor the patient and ensure their needs are met. The nurses also establish communication channels with healthcare providers and identify relevant community health resources. This program encompasses up to four consultations per year, with a maximum of 16 hours of support.

The Nursing Case Management and Delegation program goes a step further by training and supervising staff members, such as home health aides, and providing ongoing monitoring and support. This approach ensures that the personalized care plan remains effective and adaptable to changing needs. The program includes regular nursing visits every 45 days.
