Maryland DDA Self Direction Support Broker: LTSS & ISAS Services Announced

Maryland-based Community Health and Education Services (240-755-5591) announces certified support broker services for individuals choosing to self-direct under the Development Disabilities Administration.

The self-direct model was introduced to allow a greater degree of independence, choice, and flexibility for those who receive Medicaid support under the developmental disabilities program. As part of the recent update, the agency offers certified nursing support services in two categories: nursing consultation and nursing case management/delegation.

More details can be found at https://www.communityhes.org/

In addition to support brokers, Community Health and Education Services offers Long-Term Services and Support (LTSS) as part of the Community First Choice program, and also works under the In-Home Supports Assurance System (ISAS). The agency’s skilled nurses are available in both Montgomery and Prince George’s County.

“Our services are designed to help people with developmental disabilities thrive in their own setting,” a company representative explained. “To accomplish this, we consult with you and your Primary Care Provider to create a customized plan of care. Our professionals are trained to assist clients with cerebral palsy, spina bifida, and those on the autism spectrum.”

As part of the delf-directed service delivery model, participants can exercise greater control over who provides support services and how allocated funds are spent. Unlike traditional service delivery, the program includes some responsibilities, such as being the ‘employer of record,’ and having authority over budgets.

With its support broker capabilities, Community Health and Education Services offers the option of trained and certified nurses who can provide information, coaching, and advice. An in-depth guide on the agency’s website now explains the difference between nursing consultation and nursing case management, and how participants can decide which is best for their own circumstances.
